Role overview

Key responsibilities
- Administer medications and treatments per physician orders and facility policy.
- Monitor resident conditions and report changes to RN supervisor or physician.
- Assist with ADLs, wound care, and documentation under RN oversight.
- Support admissions, discharges, and care transitions as assigned.
- Maintain professional conduct with residents, families, and staff.
Qualifications & requirements
- Active NYS LPN license.
- Minimum six months of clinical experience in LTC or assisted living.
- Current BLS certification.
- Experience with electronic MAR systems preferred.
- Ability to pass background check and drug screening.
